We run a website and app called Annual Health Checks.
This helps GPs and health staff to do Annual Health Checks for people with learning disabilities.
In an Annual Health Check a GP or nurse will check your health and tell you how to stay well. These checks happen every year.
Annual Health Checks works through the website www.hearmenowchecks.com
And you can use the Annual Health Checks app on smartphones and tablets.
When you use the website and app we keep personal information about you.
This is information that someone can use to find out who you are.
Like name, date of birth and phone number.

This booklet tells you what personal information we keep when you use the Annual Health Checks website or app.

Like when your GP makes a Health Action Plan for you.
3. From the website and app
The Annual Health Checks website and app keep information about how long you use it.

How do you use or share my personal information?
We use your personal information so the Annual Health Checks website and app can help your GP.
We might share your personal information with
1. Your GP, carers or social workers
They can visit the website or app and check your Health Action Plans.
2. Other organisations we work with

How long will you keep my information for?
We will keep your personal information for as long as your GP uses Annual Health Checks.
If your GP stops using Annual Health Checks we will delete your personal data.

But we might keep your personal information to help us if you complain or
are not happy with something.
Or we might keep it if the law says we have to.
Will you send my personal information outside the UK?
We do not normally send personal information to anyone outside the UK.
But sometimes we might need to send personal information to an organisation
